TERMS OF SERVICE

  • All appointments for injectables are IN PERSON. All appointments related to hormone replacement are VIRTUAL. Please ensure you have booked correctly. If unsure, please book a FREE consultation or FREE discovery call!

    We do not provide transgender hormone replacement therapy or related medical care. For assistance with transgender health services please consult a licensed health care provider who specializes in this area.

  • We have a 24-hour Cancellation / No Show Policy in place to ensure patients can get in for appointments in a timely manner and to respect our providers’ time. Any patient that ‘no-shows’ and/or cancels within 24 hours are subject to a $100 fee charged to the card on file. All client’s that no-show without proper notification, will be required to call to book and pay a deposit of $150 in order to secure their next apt.

  • We do not offer refunds on any services rendered both aesthetic and wellness.  Aesthetic and BHRT results are quite variable from person to person and while we do our best to achieve the desired outcome, it cannot be guaranteed.  Clients are responsible for additional treatments and continued appointments to attempt to achieve desired results.

    Training: We do not offer refunds on training days unless mutually agreed upon between parties. 

    No refunds on Gift Certificate purchases.

  • We strongly recommend seeing your health provider for all your regular routine physical exams and screening, including, but not limited to: physicals, PAP tests, mammograms, and prostate exams. The NP may decide to postpone your hormone therapy until certain health assessments are completed. It is strongly encouraged to complete all other healthcare with your current provider, including prescription medication's, unless otherwise specified by our NP. This will be communicated with you prior to your initiation appointment.

  • At rebalance medical, we are committed to providing compassionate, personalized medical care with passion and integrity. We believe that a respectful, open, and collaborative relationship between our providers and patient is essential to achieving the best health outcomes.

    Respectful Behaviour:

    We kindly ask all patients approach their care with respect, openness, and a positive attitude. Disrespectful or rude behaviour will not be tolerated. We reserve the right to refuse service or discontinue care if we feel that a respectful and environment cannot be maintained.

    Client Engagement:

    To ensure thorough and effective consultations, clients are asked to be present, focused, and open minded during appointments. Please avoid booking appointments when you are feeling, rushed or distracted, as there is important information to discuss and process.

    Provider Commitment:

    Our nurse practitioner comes to each appointment with full passion, heart and dedication to practice and patient safety. We ask our clients to reciprocate this commitment by engaging fully and respecting the time and effort we invest in your care.
