Begin your BHRT journey with a detailed intake process. After booking, you’ll receive your health history form by email. Fill this out at home and our medical team will review it on the day you book. Once reviewed, a customized lab req will be sent to you.

Bloodwork:
Please complete your fasting bloodwork at your earliest convenience within 8 weeks of receiving the req.

During this visit, our Nurse Practitioner will thoroughly review your history and lab results with you and tie everything together to create a treatment plan for the first 3 months. Your prescription will be sent to one of our partner hormone pharmacies and we will send you your customized lab req for your next appointment.

Book this appointment if you have completed your Step 2: Initiation Visit with our NP are due to follow up with our Hormone RN. Follow-up visits are completed with our RN in collaboration with our NP to review your symptoms, bloodwork results and BHRT plan, and to make any necessary treatment or prescription adjustments for optimization.

*Please complete any requested bloodwork at least 2 weeks prior to this appointment.

3 month F/U + Rx = $150

6 month F/U + Rx = $250

12 month F/U + Rx = $500

Book this appointment for new or worsening symptoms, hormone side effects, medication concerns, significant changes in health status, or if you feel your current hormone treatment plan requires reassessment. This appointment is completed with our NP, who remains the primary diagnosing and prescribing provider at ReBalance Medical.

Fees for additional urgent follow ups range from $50-$150 depending on the time required and the complexity of the discussion. Additional fees may apply for labs or prescription updates.

If you’re currently receiving BHRT from another clinic and would like to transfer your care to ReBalance Medical, book this appointment. Please have all medical records, including notes, labs, and diagnostics, faxed to 1-888-270-1772 at least 1–2 weeks prior to your visit.

• Our services are not covered by provincial health plans, but may be eligible for reimbursement through a Health Spending Account (HSA) if you have one. You may also try submitting under nursing services if that applies to your plan.

• Laboratory testing ordered as part of your BHRT care is covered by provincial health coverage.

• We do not offer saliva testing, as it is considered less accurate and reliable compared to serum (blood) testing.

• If additional lab work or diagnostic imaging is required in relation to your BHRT treatment, our NP may order it directly for an additional fee. For unrelated concerns or broader diagnostic needs, we will refer you back to your family physician or primary care provider.

• Prescription refill requests outside of routine follow-up appointments must be directed through your pharmacy. Please contact them directly and ask them to send our office a refill request or discuss an interim supply with the pharmacist if needed. Many times they can provide a 2-4 week interim supply until your follow up appointment.

• All medical health and prescription inquiries must be discussed during scheduled appointments. To protect your privacy and provide appropriate medical care, we do not discuss personal health information, symptoms, medication changes, or lab results via email.
