This is not an in-person or virtual appointment.

Begin your Hormone & Wellness journey with a detailed intake process. After booking, you will receive your health history form by email. On your booked date, our medical team will review your completed form, send your lab requisition, and charge your card on file.

Bloodwork:
Must be completed fasting at your earliest convenience within 8 weeks of receiving the req.

Book after completing Step 1 and your bloodwork was done at least 2 weeks ago. Our NP will review your bloodwork results, discuss your treatment plan, and issue a 3-month prescription & follow-up lab req.

Book this appointment if you have completed Step 2: Initiation Visit with our NP and are due for follow-up with our Hormone RN. F/U visits are completed with our RN in collaboration with our NP to review your symptoms, bloodwork results, treatment response & to make any necessary treatment adjustments for continued optimization. During your 1st year of treatment, f/u visits are completed every 3 months. After your 1st year, f/u frequency and prescription duration are determined by the NP and may be adjusted at any time based on your individual treatment needs and clinical stability.

Please complete any requested bloodwork at least 2 weeks prior to your appointment.

Cost:

3-Month Follow-Up + Prescription: $200‍ ‍

6-Month Follow-Up + Prescription: $350‍ ‍

12-Month Follow-Up + Prescription: $600

Book this appointment for new or worsening symptoms, concerning side effects or changes in your health that may require clinical assessment before your next scheduled follow-up. This is not intended for routine questions, minor/expected outcomes or prescription refills.

• Our services are not covered by provincial health plans, but may be eligible for reimbursement through a Health Spending Account (HSA) if you have one. You may also try submitting under nursing services if that applies to your plan.

• Laboratory testing ordered as part of your BHRT care is covered by provincial health coverage.

• We do not offer saliva testing, as it is considered less accurate and reliable compared to serum (blood) testing.

• If additional lab work or diagnostic imaging is required in relation to your BHRT treatment, our NP may order it directly for an additional fee. For unrelated concerns or broader diagnostic needs, we will refer you back to your family physician or primary care provider.

• Prescription refill requests outside of routine follow-up appointments must be directed through your pharmacy. Please contact them directly and ask them to send our office a refill request or discuss an interim supply with the pharmacist if needed. Many times they can provide a 2-4 week interim supply until your follow up appointment.

• All medical health and prescription inquiries must be discussed during scheduled appointments. To protect your privacy and provide appropriate medical care, we do not discuss personal health information, symptoms, medication changes, or lab results via email.
