This service is for clients paying out of pocket or those with a health spending account through their employer.
Alyssa, RN provides treatment exclusively under these payment arrangements, as insurance billing is not available.

This service is not billed through insurance.
You may be able to submit your receipt for reimbursement if you have a health spending account.

Important:
Before booking, please contact your insurance provider to confirm coverage. You will need to provide the DIN#01981501.
If your insurance does not cover the treatment, you will be responsible for the cost out of pocket.
During this consultation, we will answer your questions, complete all necessary paperwork, and submit your prescription to BioScript pharmacy.
If your treatment is covered by insurance, no injections will be performed during this appointment.
An injection fee of $250 applies on the day of treatment and is not covered by insurance.
